In this session each attendee will understand the key elements to establishing a patient navigation program. During this session we will identify at least 2-3 metric’s to use for outcomes around navigation and adherence . During this interactive workshop session, attendee’s will help develop how to track outcomes and use this information to support the patient navigation program to your practices, patients and payors.
